  • Shared Savings (When you seek health care outside the UnitedHealthcare network, your resulting out-of-pocket costs will generally be higher. However, when you receive health care from physicians and facilities which are part of the Shared Savings program, your claim will still be paid at the out-of-network benefit level, but at a discounted rate for covered expenses. This will be used to determine the amount of your out-of-pocket cost. Also, you will not be billed the portion of the charges that exceeds the discounted rate.)
Cancer Resource Services

Insureds that have chosen a Golden Rule Insurance Company health insurance plan with access to UnitedHealthcare Networks (Choice Plus or Options PPO) have access to a selection of elite cancer centers across the country known as the Cancer Resource Services (CRS) Centers of Excellence (COE) Network. Each center meets extensive evaluation criteria annually to ensure that care is being provided by centers that can quantify their expertise. Access is available to all of the centers listed below. If you will be visiting one of the facilities with an asterisk by its name (in blue), please call
(866) 936-6002 before your first visit; otherwise, follow standard procedures just as you would when visiting any UnitedHealthcare network provider. Please note: network subject to change.

Transplant Services 

Centers of Excellence facilities specialize in specific types of transplants. These facilities have agreed to provide quality care on a cost-efficient basis. Insureds are encouraged to utilize Centers of Excellence whenever possible.
